Tiffany Chohfi MSN, RN is a Project Director at the Colorado Center for Nursing Excellence. Tiffany has been a registered nurse since 2014 and received her Master of Science in Nursing leadership and education degree from Walden University.
Tiffany leads the NEPQR-BHI grant, funded through HRSA. This grant is focused on implementing behavioral health specialists in primary care clinics in rural Colorado. Through creative innovation, evidence-based models and practice, Tiffany leads primary care clinics to full behavioral health integration through recruiting behavioral health consultants (BHC’s), EHR building, team and practice transformation, training, coaching and supervision of clinic goals and sustainability. This has led to an abundance of mental health access in rural communities of Colorado leading to better patient outcomes and equitable care for children and adults in rural and underserved areas.
She is also the Project Director on HRSA’s SANE (Sexual Assault Nurse Examiner) Grant in which she assists 6 surrounding states recruit, retain, and educate nurses to become SANEs and leads education and training partnerships to aid nurses in becoming sexual assault nurse examiners to evaluate and treat victims of sexual assault and violence. She has recruited, created, and provided resources for over 120 new Sexual Assault nurse examiners in Colorado and surrounding states leading to an increase of access to care for underserved populations.
