Mountain Park Health Center (MPHC), a large FQHC with 11 clinics in the Phoenix metropolitan area, serves approximately 113,000 patients annually (UDS, 2024). MPHC was a pioneer in incorporating integrated behavioral health into primary care over 20 years ago. Many health care settings aspire to provide integrated care but often face implementation and execution challenges. Come learn how we have successfully implemented the PCBH model into our clinics and how we continue to increase collaboration efforts aimed at serving our most vulnerable populations.
