This session will provide a structured approach to documenting and evaluating the implementation of CoCM through the use of case studies. By the end of this session, attendees will have the skills and resources to document and share CoCM implementation successes and challenges through well-structured case studies. These case studies will serve as valuable tools for research, evaluation, and quality improvement efforts, ultimately advancing CoCM adoption and effectiveness.
