Description

This session will describe the inception and growth of our Behavioral Health Care Coordination (BHCC) program within an integrated care setting. We will discuss the wins and learning opportunities we have encountered through program development. We will highlight the importance of the BHCCs and how they have helped to elevate the work of the Behavioral Health Consultation team in primary care.
