Community primary care teams develop treatment plans, but much of chronic care is carried out at home by patients and their families. This session presents an emerging family-driven care model that combines human health coaching with multi-person voice AI to support adherence, identify barriers, and maintain continuity between primary care visits. Using implementation experience from Resolve Together’s collaboration with a community clinic, presenter will describe how the model fits into existing community primary care workflows, engages family caregivers without adding work for clinic staff, and generates actionable information for the care team. The session will also examine early implementation measures, including patient enrollment, family engagement, goal completion, care-plan follow-through, and operational feasibility. Participants will leave with a practical framework for determining when and how family-inclusive technology can strengthen integrated chronic care.
