Jacqueline Fuentes, MSW, LCSW is Behavioral Health Consultant with UNC Health Family Medicine & Pediatrics, where she helped implement an innovative Primary Care Behavioral Health (PCBH) pilot integrating behavioral health services into primary care. Since beginning her career in behavioral health in 2008, she has worked across psychiatric residential treatment, school social work, care management, and behavioral health leadership, developing innovative programs and addressing gaps in care for children, adolescents, adults, and families. Jacqueline is passionate about creating practical, family-centered approaches that improve access to behavioral healthcare and strengthen collaboration across healthcare, education, and community systems. She enjoys translating clinical experience into sustainable models that improve outcomes for patients, providers, and communities.
