Matthew Hoag, MA, LPC, LAC, is Director of Integrated Behavioral Health at Denver Health, where he leads a large interdisciplinary behavioral health division embedded across the organization’s safety-net primary care system. His work focuses on advancing sustainable integrated care models, strengthening clinical operations, supporting the behavioral health workforce, and demonstrating the value of team-based care.

Matthew serves as Co-Chair of the Colorado Behavioral Health Advisory Network and is the principal investigator for an Anschutz Foundation initiative expanding the Primary Care Behavioral Health model across Denver Health’s school-based health centers. His professional interests include integrated care implementation, workforce development, health equity, and building systems that enable patients, teams, and communities to thrive.
