This presentation will share the story of one organization’s journey to implement Measurement Based Care (MBC), with a focus on the unique role of supervisors in the implementation and fidelity of the practice. Based upon best practices in the literature as their own creative approach to data-informed decision-making throughout the first 2 years of their MBC journey. Leaders from Eliot Community Human Practice’s multidisciplinary team will share stories and aggregated data highlighting the strategies they used at the supervisory and leadership level to achieve: -Increasing patient engagement in completing MBC assessments -Enhancing clinician adherence to MBC practices -Improving patient outcomes through structured supervision and data utilization -Strengthening program sustainability by integrating MBC across multiple service lines
