Description

Implementing a new behavioral health model within a well-established primary care behavioral health system has been both a rewarding and challenging experience. This session will provide insights into our journey of integrating the Collaborative Care Model (CoCM) with a large, complex health system. From forging key partnerships to creating an implementation-ready infrastructure, participants will gain practical strategies and actionable steps to apply in their own CoCM initiatives.

Objective 1
Understand the implementation journey--describe the process of integrating CoCM within an established primary care behavioral health program, including key challenges, successes and lessons learned.
Objective 2
Explore practical implementation strategies--examine the project plan, identify key stakeholders, consider adaptations in Epic, and identify advocacy efforts to consider for access to billing codes.
Objective 3
Analyze outcomes and future directions -- review preliminary data on the implementation of CoCM, discuss its impact and identify next steps for continued growth and improvement.
