By examining how these two approaches differ and identifying the optimal settings for their implementation, this presentation aims to provide clarity on the appropriate integration of each model in healthcare systems.

Implementing the Collaborative Care Model (CoCM) at Baptist: Challenges, Insights, and Lessons Learned
Objective 3
The integrated care section will examine and showcase a multidisciplinary integrated care framework structured to meet the psychological, emotional, relational, and existential needs across the lifespan. Drawing on the early implementation of a program focused on older adults, the session will highlight strategies to reduce fragmentation, enhance patient-provider connection, and restore continuity, dignity, and meaning across care systems.
