Participants will present and refine their Measurement-Based Care Implementation Blueprint, a practical plan that includes:

• Selected outcome measures
• Data collection workflow
• Strategies for patient engagement
• Leadership and team supports
• SMART implementation goals

Participants will receive peer and facilitator feedback and identify next steps for implementation in their organizations. The session concludes with reflection on lessons learned and strategies for sustaining momentum after the course ends.
