Diana Rivera-Burgos MSW, LCSWA has been a provisionally licensed social worker since 2023. After obtaining her Bachelor’s degree in Social Work from North Carolina State University in 2012, she graduated from the Simmons University MSW Program in 2020. She has worked in a Primary care setting using the Primary Care Behavioral Health (PCBH) Model since 2018. She is currently a Behavioral Health Care Manager at Piedmont Health Services in Siler City. In this role, Diana provides care coordination and case management for patients in the Psychiatric Collaborative Care Management (Psych CoCM) program, which includes conducting screenings and clinical assessments, treatment planning, providing brief evidenced based interventions, providing support with medication management, and facilitating communication with treatment team regarding treatment changes. Diana is grateful for the opportunity to serve such diverse population of people within her organization. She is motivated to improve access to behavioral health care in underserved populations. She strives to ensure that her patients receive high-quality, culturally aware, impactful care.
