Aligning Incentives for Whole-Person Care: Lessons from California’s Behavioral Health Integration Payer Collaborative

The California Quality Collaborative (CQC), part of the Purchaser Business Group on Health, has been convening a Behavioral Health Integration (BHI) Payer Workgroup in 2026 to accelerate adoption of integrated behavioral health and primary care across California. While many health systems and primary care organizations are implementing integrated care models such as the Collaborative Care Model (CoCM) and Primary Care Behavioral Health (PCBH), payment and data infrastructure within commercial health plans often lag behind clinical innovation. Commercial payer policies can unintentionally create barriers to implementation of behavioral health integration through fragmented payment models, limited data infrastructure, and misaligned incentives. At the same time, health plans play a critical role in scaling integrated care through value-based purchasing, network requirements, and strategic data use.
